This form must be submitted by 11:59 PM on Monday November 4, 2024. Please note if you select the paypal option to pay using paypal or a credit or debit card, once you fill out and submit the form, and continue from the review screen, you will be redirected to pay pal to complete your payment using a paypal account or a debit or credit card. If you encounter issues with pay pal, we at the national federation of the blind of Ohio cannot resolve these issues for you.
